Hospice Nurse Liaison (Manhattan)
For over 125 years, Calvary Hospital and Calvary @Home located in the Bronx, New York has been dedicated to the palliative care of advanced cancer and other terminally ill patients and their families. Through a caring, compassionate philosophy of non-abandonment, the hospital provides a full spectrum of palliative services, including emotional, spiritual, and physical care. The hospital receives patients from referring institutions throughout New York City and the entire tristate area. Requirements
Qualifications:
- Current New York State Registered Nurse License/Permit.
- Current valid NYS Driver’s License preferred.
- Must have own car to conduct site assessments.
- Minimum of three years relevant clinical experience.
- Working knowledge of inpatient/out-patient/home care/hospice; application of admission/continued stay criteria and third party payers.
Education:
- Bachelor’s degree preferred.
Responsibilities:
The Hospice Nurse Liaison are responsible for coordinating applications through the admission review process. Coordinates and maintains referrer relationships with assigned institutions/agencies. Participates in development and marketing Calvary Hospital’s Home Care and Hospice Services. Provides pre-admission assistance to patients, families and referrers.

Salary Range:
- $109,208 - $135,708
