Opportunity Highlights
- Part-time post-acute care position in a skilled nursing / inpatient setting
- 2 days per week schedule: Tuesdays and Thursdays
- Primary care–focused role with flexibility and strong work-life balance
- Manage both acute and chronic conditions for post-acute patients
- Develop individualized care plans and collaborate closely with the interdisciplinary care team
- Work in partnership with an on-site physician for clinical support
- Well-structured onboarding and strong back-office infrastructure

Compensation & Benefits
- Fee-for-service (productivity-based) compensation model
- Estimated annual compensation range: $49,096 – $58,917 , with opportunity for incentives
