Social Worker - Partners in Parenting Program
New Alternatives for Children, Inc. (NAC) is an award-winning not-for-profit agency in Midtown Manhattan dedicated to serving children and families with medical complexities, chronic illnesses, and disabilities throughout the five boroughs. To assist in achieving this mission, NAC is currently seeking a Social Worker/Mental Health Counselor/Case planner to join its Partners in Parenting program.
The Partners in Parenting (PIP) program is an aftercare program that serves families that have successfully transitioned out of preventive services and foster care child welfare programs. The goal of this program is to prevent a family’s repeat involvement in child welfare by helping families remain intact while ensuring caregivers receive the appropriate support to meet all of their children’s physical, developmental, educational, social/emotional, and mental health needs. The ideal candidate will be strength-based, trauma-informed, culturally responsive and committed to family preservation and resilience. This is a home and community-based program so the candidate must be flexible with the ability to work independently and flex their hours to meet the needs of youth and parents. We operate on a hybrid model allowing for work-from-home when not in the field. - Salary: Master’s Level - $60,000 - $65,000 / Bachelor’s Level - $50,000 - $55,000 (Additional compensation for bilingual skills (English/Spanish) and licenses (LMSW, LMHC, etc.))

Essential Functions:
- Provides family-centered, case management and clinical services utilizing individual, family and group work modalities.
- Conducts home and community-based visits to families based on case acuity.
- Ensures adherence to agency and programmatic policies and regulations; completes all required documentation in our electronic health record system including service plans and progress notes.
- Provides comprehensive psychosocial needs assessments to determine individualized and family services and interventions.
- Participates as a team member in monthly case reviews.
- Coordinates services as recommended by NAC’s interdisciplinary treatment team which includes our psychiatrist, psychologists, medical doctor, nurse practitioners, educators, and resources, advocacy and homelessness prevention services.
- Provides care coordination with doctors, schools, mental health clinics and community-based organizations.
- Provides information, referral and advocacy services.
- Attends weekly individual supervision, on-site clinical and mental-health training and social justice discussions.
- Candidates receive credit toward NYS LCSW and LMHC licensing requirements. Free CEUs are available to licensed social workers (LMSW/LCSW) along with licensing prep courses for MSWs and MHCs who have completed probation.
Required Education and Experience:
- Master's degree in social work, mental health counseling or in case management (with applicable experience).
Minimum Requirements:
- Some knowledge of health-related factors in working with children with disabilities, medical complexity and behavioral health conditions.
- Prior experience in child welfare, mental health, health care settings or case management.
- Work hours are generally 9-5pm; must be flexible to meet the needs of working parents.
- Proficiency in Spanish is a plus; competitive salaries.
