Physical Therapist Job Details

• End Date: December 22, 2025
• Contract Duration: 13 weeks
• Shift: Day Shift, 8:30 AM – 4:30 PM
• Hours per Week: 40
• Shifts per Week: 5
• Shift Duration: 8 hours
• Rotating Weekends: Yes
• Caseload: 5-7 home health cases per day
• Mileage: Reimbursed at IRS rate
• Estimated Weekly Salary Range: $2,500 - $2,560

Physical Therapist Qualifications

• Active New York State Physical Therapist license (required; NY is not a compact state)
• BLS certification (AHA required)
• Minimum 1 year of experience as a Physical Therapist
• Home health experience preferred
• Epic charting system experience preferred
• COVID-19 vaccine required (medical exemptions only)
• Ability to travel to assigned areas across Long Island, NY

Physical Therapist Responsibilities

• Review patient charts and medical history for home health cases
• Develop and implement personalized treatment plans for adult and geriatric patients
• Teach therapeutic exercises and stretches to improve patient outcomes
• Provide hands-on physical therapy to promote healing and recovery
• Maintain accurate patient records, track goals and progress, and adapt treatment plans as needed
• Collaborate with an interdisciplinary healthcare team including physicians, nurses, social services, pastoral care, and home health aides
• Travel to patient homes throughout Farmingdale and Long Island regions
