Physical Therapist Job Details

• Location: Webster, NY 14580 (Monroe County)
• Healthcare Setting: Hospital / Home Health
• Employment Type: Locum Tenens (Travel Physical Therapist Contract)
• Duration: 13 weeks (Start: 09/15/2025 – End: 12/13/2025)
• Schedule: Monday – Friday, 8:00am – 4:30pm; weekends as needed
• Hours per Week: 40
• Shift Duration: 8 hours
• Mileage: Reimbursed
• Estimated Weekly Salary Range: $1870-$1920

Physical Therapist Qualifications

• B.S., M.S., or D.P.T. degree in Physical Therapy
• Active New York State Physical Therapy License
• BLS certification (AHA only)
• Minimum 1 year of experience as a Physical Therapist in home health or long-term care
• Recent home health experience (within the last year)
• Strong verbal and written communication skills
• Ability to physically lift and assist patients in direct care
• Demonstrated ability to work effectively with diverse socio-cultural backgrounds

Physical Therapist Responsibilities

• Provide case management and direct patient care as part of a multidisciplinary healthcare team
• Administer comprehensive physical therapy modalities in home health and hospital settings
• Collaborate with physicians to evaluate patient function and develop/revise care plans
• Prepare clinical and progress notes; consult with families and agency personnel
• Educate patients, families, and team members on disease processes, self-care, and preventive strategies
• Implement home therapy programs (gait training, transfers, ADLs, home modifications, equipment use)
• Supervise Physical Therapist Assistants (PTAs) and Home Health Aides to ensure patient safety and compliance
• Participate in interdisciplinary team meetings and in-service programs
• Maintain accurate clinical records and ensure compliance with New York state and federal healthcare regulations
• Promote positive communication and relationships with patients, families, and staff
• Contribute to continuous improvement and quality management initiatives
