Job Title: Registered Nurse (RN) – Transplant (Travel Assignment)

Location: New York, NY

Duration: 13 Weeks

Schedule: Day Shift | 3x12-Hour Shifts (07:00 AM – 07:30 PM)

Weekly Travel Pay: $2,575 – $2,801

Position Overview

"Navitas Healthcare, LLC" is seeking an experienced Registered Nurse (RN) to support a transplant program in an outpatient and clinical coordination capacity. This travel assignment focuses on delivering comprehensive patient care, including triage, lab follow-up, clinic coordination, and patient education. The RN will collaborate closely with physicians and advanced practice providers to ensure continuity of care for transplant patients.

Key Responsibilities

Triage & Care Coordination

• Triage patient calls and communicate needs to providers
• Collaborate with interdisciplinary teams to ensure timely patient care

Laboratory Review & Follow-Up

• Review outpatient lab results with providers
• Communicate medication updates and care plans to patients and caregivers
• Ensure timely follow-up and continuity of care

Clinic Support

• Assist during clinic visits alongside providers
• Perform medication reconciliation and review of systems
• Coordinate follow-ups, lab work, and treatment plans

Patient & Family Education

• Educate patients and caregivers on transplant care and medications
• Provide clear, compassionate guidance throughout treatment

Administrative Support

• Manage communications (calls, faxes, documentation)
• Complete required forms and assist with prior authorizations

Required Qualifications

• Active Registered Nurse (RN) license
• Experience in transplant, specialty clinic, or acute care setting
• Strong clinical assessment and coordination skills
• Excellent communication and patient education abilities
• Ability to work effectively in a multidisciplinary team
