Community health worker
We are seeking a compassionate individual to provide assistance in accessing medical care and support services for low-income women and children with HIV.
Title: Community Health Worker - HIV Part D
Overview: The Community Health Worker provides community-based case management, health education/risk reduction, psychosocial support and referrals to community partners to reduce barriers to the social determinants of health causing issues with attendance at medical appointments and with medication adherence.
Pay Range : $44,000 - $46,000 Annually depending on experience
Status: Full Time (37.5) Hourly. Monday to Friday. Benefits Eligible.
Qualifications :
1. Bachelor’s degree required in social work, psychology, or closely related field. Equivalent experience may be substituted for a degree.
2. Minimum of 2 years related work in a health or human service field preferably with an HIV/AIDS and/or other chronic illness organization.
3. Experience in providing strength-based, client-centered, proactive care coordination/case management.
4. Outgoing and professional interpersonal, verbal, and written communication skills and ability to interact with a wide range of individuals.
5. Availability to work a flexible schedule with some evenings and weekends.
6. Proficiency in MS Word/Excel/Outlook. Familiarity with client services databases such as HMIS and AIRS a plus. Use of Electronic Health Record databases and referral platforms is also required.
7. Valid, unrestricted driver's license and independent means of transportation.
Responsibilities:
- Provide outreach and community-based case management services to low-income women, children and youth with HIV to reduce barriers to the social determinants of health.
- Provide health education/risk reduction information to improve medical care adherence.
- Provides psychosocial support that addresses mental, emotional, and social needs of clients.
- Develop and maintain a thorough knowledge of eligibility and benefit provisions for health care and other supportive services. Facilitate referrals for services and assist with enrollment in appropriate programs as necessary.
- Escort clients to appointments as needed.
- Provide time-sensitive activity reports and collect data to support the measurement of program outcomes.
- Ensure compliance with Article 27F HIV Confidentiality Law and HIPAA confidentiality for all interactions.
- Attend full staff meetings and meetings required 24 hours of annual training.
- Other duties and special projects as assigned.
