Licensed Master Social Worker - Nursing Home Surveyor/Complaint Investigator - New Rochelle, NY (#25273)
Location: New Rochelle, NY (Travel to onsite locations required).
Employment Type: Full-Time, Temporary to Permanent.
Hourly Rate: $39 per hour.
Position Overview:
We are seeking a dedicated Licensed Master Social Worker to serve as a Nursing Home Surveyor/Complaint Investigator. This critical role involves conducting surveys and investigations, including allegations of abuse and neglect, at skilled nursing facilities to ensure compliance with Federal and State regulations. The ideal candidate will be a meticulous professional committed to upholding the highest standards of care for long-term care residents.

Qualifications:
- Education: Master's Degree in Social Work (MSW).
- Licensure: NYS Licensed Master Social Worker (LMSW) required.
- Experience: A minimum of two (2) years of clinical experience in a medical facility (nursing home preferred). Two (2) years of experience performing nursing home surveillance activities is highly preferred.
- Technical Skills: Proficiency with data entry in Federal and State reporting databases. SMQT Certification is preferred.
- Soft Skills: Excellent written and verbal communication, keen analytical and investigative skills, strong attention to detail, ability to work independently, and a firm understanding of regulatory compliance.
Key Responsibilities:
- Participate in surveys or complaint investigations to include allegations of abuse and neglect, and perform surveillance activities/tasks at nursing homes (long-term care) to determine compliance with Federal and State regulations.
- Produce written documentation and draft a Statement of Deficiencies (SOD) within the required time frame, using Principles of Documentation.
- Assess compliance with the Plan of Correction within the required time frame.
- Complete data entry in Federal and State reporting databases as required.
- Participate in State monitoring during Immediate Jeopardy situations.
- Attend meetings as required.
- Testify in administrative hearings as needed.
- Adhere to DOH work schedules.
- Adhere to DOH and IPRO policies, and procedures.
- Travel to onsite locations according to work schedule.
