Duties Description PRIMARY DUTIES AND RESPONSIBILITIES INCLUDE BUT ARE NOT LIMITED TO: Regional Field Office 4 is seeking a Licensed Master Social Worker for the Crisis Unit CSIDD/Community. The Crisis Unit plays a critical role in providing linkages, information, technical assistance, consultation and coordination of support and services to OPWDD individuals as well as a range of community partners in region 4. The crisis team works directly with CSIDD, provider agencies, hospital staff, local and state government partners, MHLS and others.RESPONSIBILITIES: o Support individuals in crisis for all of region 4o Review and authorize CSIDD service and resource center stays including review of referral packets and scoring and management of BPIR scoring and management. o Provide technical assistance and systems education to community partners while working alongside CSIDD/NY START providers and other community partners to ensure that the individual is being adequately supported in the community. o Interface and coordinate collaboratively with other regional office teams to ensure individual's needs are being met timely including enrollments, referrals for residential placement, etc.o Maintain and utilize both the CSIDD SharePoint system the Crisis Unit SharePoint tracking systems with real-time information, update case activity forms timely, and provide leadership with updates as necessaryo Participate in regular meetings with relevant community partners (including CSIDD, hospitals, CCOs, etc.) and follow up with agency staff as necessaryo Additional crisis team duties as assigned o Perform other tasks and duties as assigned related to RFO needs.LinkedIn: #LI-SB1

The minimum qualifications required for this vacancy. Minimum Qualifications Open Competitive: license and current registration as a Licensed Master Social Worker in New York State. Transfer: Candidates must have one year of permanent competitive or 55-b/c status in the posted title and be eligible for lateral transfer; OR be eligible to transfer via Section 70.1, 70.4, or 52.6 of Civil Service Law. HELPS Program titles may be filled via a non-competitive appointment. This means that you do not need to take an exam to qualify, but you do need to meet the minimum qualifications of the title. At a future date (within one year of permanent appointment), employees hired under NY HELPS are expected to have their permanent non-competitive employment status converted to permanent competitive status. Posted Hiring Salary includes the sum of the statutory hiring rate for PS&T Grade 20 ($74,193) and Downstate Adjustment ($4,000).
