Responsibilities
- Provide direct nursing care to residents according to individualized care plans in a long-term care setting.
- Administer prescribed medications and document administration in PointClickCare EMAR; monitor for potential adverse effects and interactions.
- Monitor and report changes in patient condition; perform routine assessments, vital signs, and daily documentation; participate in care rounds with the nursing team.
- Assist residents with activities of daily living, support dementia care plans, and help manage chronic diseases such as diabetes per protocol.
- Ensure infection prevention and control practices, maintain accurate patient records, and comply with facility policies and safety standards.

Requirements
- Current New York Licensed Practical Nurse (LPN) license
- BLS (AHA) certification
- 1–3 years of nursing experience; SNF/LTC experience preferred, but not limited to LTC
- Proficiency with EMAR systems; experience with PointClickCare is a plus
- Ability to work in a long-term care setting and support residents with dementia and chronic conditions
