Minimum Qualifications:

Education: Bachelor’s degree in human services or a related field

Experience: At least two (2) years of experience in the mental health field, including experience working with individuals in independent living settings

Work Eligibility: Must be authorized to work in the U.S.

Work Location Requirement: Onsite; requires regular travel to program sites and client residences within the community

Compensation

Salary Range: $27.3555 an hour

About the Role: The Rehabilitation Counselor supports young adults transitioning from foster care into supported housing by helping them build and maintain independent living skills. Working as part of an interdisciplinary team under the supervision of the Program Director, this role connects individuals to medical, social, vocational, and educational services while promoting recovery, self-sufficiency, and community integration. The position requires regular travel to client residences and significant independent work.

Primary Responsibilities

  1. Provide in-home and community-based supportive services to clients on a scheduled basis
  2. Assist clients in developing independent living skills, including crisis management and emotional regulation
  3. Support high-risk clients through direct casework and provide guidance to case managers
  4. Maintain accurate and timely case records and contribute to individualized service plans
  5. Carry an active caseload, including high-risk clients and oversight of additional cases
  6. Facilitate case conferences and collaborate with interdisciplinary team members
  7. Link clients to medical, mental health, and social services and advocate for needed resources
  8. Plan and coordinate recreational, social, and vocational activities
  9. Lead or co-lead support groups and community-based programming (e.g., GED tutoring, group activities)
  10. Conduct assessments of clients’ social, psychological, and medical needs
  11. Promote recovery, self-empowerment, and independence in all interactions
  12. Coordinate referrals and respond to information requests
  13. Participate in staff meetings, supervision, and ongoing training
  14. Assist with family-related services and program events
  15. Perform other related duties as assigned
