Responsibilities
- Provide direct nursing care to residents in an LTC unit, including routine assessments and monitoring.
- Administer medications and treatments per physician orders, documenting in the electronic medical record.
- Monitor vital signs, respond to changes in condition, and escalate issues to the supervising licensed nurse.
- Collaborate with the interdisciplinary team to implement care plans and ensure resident safety and comfort.
- Assist residents with activities of daily living and support memory care needs as appropriate.
- Maintain infection control practices, ensure accurate documentation, and participate in unit handoffs and rounds.
- Float within the same building as needed to support patient care coverage and staffing needs.

Requirements
- Active Licensed Practical Nurse license in New York (LPN NY)
- Current Basic Life Support (BLS) certification
- Nursys verification of licenses (active LPN NY license and any other active licenses)
- Minimum 1 year of experience in a skilled nursing facility (LTC)
- Proficiency with PointClick Care or comparable EMR system
- Ability to work the listed LTC setting and 2:30 PM–10:30 PM shift
- Local eligibility within 75 miles preferred for local rate
- RTO (time off) requests must be included at submission
- Documentation of drivers license/address matching demographics
