Qualifications:
- Active NY State Medical License.
- Board Certified or Board Eligible.
- Active Medicaid number & NPI.
- Spanish-speaking preferred.
Key Responsibilities:
- Diagnose and treat acute and chronic conditions.
- Provide preventive care, immunizations, and health counseling.
- Review labs, imaging, and adjust treatment plans.
- Coordinate care and refer to specialists as needed.
- Maintain thorough documentation.
- Support patients with long-term management of conditions.
