Growing Practice Seeks Pain Management Provider (PA/FNP). Middletown Medical is a very successful, extensive, multispecialty practice located in the beautiful Hudson Valley Region of New York, only 1 hour and 15 minutes North of NYC. Since 1984, we have been a physician-owned, physician-led practice, providing our patients with a comprehensive care plan that integrates our specialists to achieve the best outcomes. We currently have an open position for a knowledgeable Pain Management providers to join our growing team of compassionate providers. Middletown Medical’s Pain Management & Rehabilitation Department offers a wide range of treatments to provide the best possible pain management and improve patients' physical, psychological, and social functioning. Our talented physicians and staff include those trained in physical medicine and rehabilitation, pain medicine, interventional pain management, and sports medicine. We provide a full range of on-site diagnostic testing tools, as well as counselors and support staff with unique insight in this field. Our goal is to partner with our patients during difficult times and help them return to an active, productive lifestyle.

About the Practice
Middletown Medical is a multidisciplinary pain management practice dedicated to delivering evidence-based, patient-centered care for acute and chronic pain conditions. Our team includes pain physicians, advanced practice providers, physical therapists, and behavioral health professionals working collaboratively to improve patient outcomes and quality of life.
Responsibilities:
- Perform comprehensive pain assessments, including medical history, physical examination, and functional evaluation.
- Develop and implement individualized pain management treatment plans.
- Manage chronic pain conditions using pharmacologic and non-pharmacologic therapies.
- Prescribe medications in compliance with state and federal regulations, including controlled substances.
- Monitor patient outcomes, medication efficacy, and adverse effects.
- Provide patient education regarding pain conditions, treatment options, and expectations.
- Coordinate care with physicians, therapists, and other healthcare professionals.
- Maintain accurate and timely documentation in the electronic medical record (EMR).
- Comply with regulatory, ethical, and practice standards, including opioid stewardship guidelines.
Qualifications Required:
- Current Nurse Practitioner license (ANCC or AANP certified).
- Active state NP/PA license and DEA registration (or eligibility).
- Strong clinical assessment and decision-making skills.
- Excellent communication and patient counseling abilities.
Preferred:
- Prior experience in pain management, orthopedics, PM&R, neurology, anesthesia, or primary care.
- Familiarity with chronic opioid management and interventional pain workflows.
- Experience with EMR systems.
- Certification or training in pain management (highly desirable).
