TITLE: PMHNP Walton

Hours: M-F Weekend Rotation

SUMMARY: providing PMHNP care in an out patient clinic setting

To work as part of an interdisciplinary team providing direct patient care, consultation and supervision in a collaborative effort to identify and treat patients. The psychiatrist provides comprehensive psychiatric care, including diagnostic interviews, medication, medication reviews, and therapy to patients.

DUTIES:

Conduct comprehensive psychiatric evaluations of patients presenting with various mental health concerns.

Perform thorough assessments of patients' medical history, mental status, and psychosocial factors to develop individualized treatment plans.

Prescribe and manage psychotropic medications for patients with psychiatric disorders, following evidence-based guidelines and best practices.

Monitor medication efficacy and side effects through regular follow-up appointments and adjust treatment plans as necessary.

Provide evidence-based psychotherapy, such as cognitive-behavioral therapy (CBT), dialectical behavior therapy (DBT), and supportive therapy, to individuals and groups.

Offer counseling and support to patients and their families to address emotional, behavioral, and interpersonal issues.

Collaborate with other healthcare providers, including psychiatrists, psychologists, social workers, and primary care providers, to coordinate comprehensive care for patients.

Consult with interdisciplinary teams to develop holistic treatment plans that address the biological, psychological, and social aspects of mental illness.

Educate patients and their families about mental health conditions, treatment options, medication management, and strategies for self-care.

Provide health promotion and prevention services, including psychoeducation on stress management, coping skills, and healthy lifestyle choices.

Maintain accurate and up-to-date medical records, including progress notes, treatment plans, and medication prescriptions, in compliance with legal and ethical standards.

Ensure confidentiality and privacy of patient information in accordance with HIPAA regulations.

Conduct risk assessments for patients experiencing psychiatric crises or exhibiting suicidal or homicidal ideation.

Implement appropriate interventions, including safety planning, crisis stabilization, and referral to emergency services as needed.

Participate in quality improvement initiatives to enhance the delivery of mental health services and improve patient outcomes.

Monitor and evaluate treatment outcomes through outcome measures, patient satisfaction surveys, and clinical data analysis.

Stay current with advances in psychiatric practice, pharmacotherapy, and psychotherapy through continuing education, conferences, and professional literature.

Pursue certification and licensure renewal requirements to maintain competency and licensure as a PMHNP.

These duties encompass the core responsibilities of a PMHNP in an outpatient clinic setting, where they play a vital role in providing comprehensive mental health care to individuals across the lifespan.

QUALIFICATION:

BSN and MSN Graduate from an accredited Nurse Practitioner Program

License: Registered Nurse in New York State

Board Certified

Certification: Nurse Practitioner Certification & Registration, Certified or eligible for certification by recognized specialty organization, Current practice agreement

Minimum 2 years of nursing experience and one year as a Nurse Practitioner.

Demonstrated organizational, clinical, and interpersonal skills, as well as ability to problem solve using critical thinking

Demonstrated basic computer skills including typing proficiency.

Bilingual English-Spanish preferred.
