Requirements
- Active LPN License & Registration (NY)
- BLS/ CPR
- Updated Resume
- Valid ID & Social Security Card
- Physical Exam (within 1 year)
- PPD/Quantiferon (within 1 year)
- MMRV (Measles, Mumps, Rubella, Varicella)
- Covid Vaccination Card
- Voided Check or Direct Deposit Form
- 2 Reference Letters
