Nurse Care Manager

Compensation: $105K–$115K per year + $20K SOB

Job Type: Field-based with regional travel; Monday through Friday schedule. Hybrid 4 days at home 1 in the Field.

Location: Bronx, NY

Our client is seeking a compassionate and experienced Nurse Care Manager to join their Managed Long Term Care (MLTC) team. This role is ideal for an RN who is passionate about supporting older adults and helping members live independently in their homes for as long as possible.

As a Nurse Care Manager, you’ll play a key role in coordinating care and ensuring quality outcomes through assessment, planning, facilitation, and advocacy.

Key Responsibilities

• Conduct in-home assessments and develop individualized care plans based on members’ medical, social, and functional needs.
• Collaborate with members, caregivers, and families to promote independence and optimal quality of life.
• Coordinate care across providers, payers, and community resources to ensure continuity and cost-effective outcomes.
• Authorize covered services in accordance with plan policies and guidelines.
• Communicate regularly with the interdisciplinary team, including primary care practitioners and specialists, to monitor progress and address changing needs.
• Serve as an advocate for members, ensuring they receive appropriate services and support.

Qualifications

• Active New York State Registered Nurse (RN) license and registration.
• Associate’s degree in Nursing required; Bachelor of Science in Nursing (BSN) preferred.
• Minimum of 3 years of RN experience.
• Prior experience in Managed Long Term Care (MLTC) required; geriatrics experience preferred.
• Bilingual in Cantonese, Mandarin, Spanish, or Russian required.
