Status: Part-Time, In-Person

Reports To: Program Director

Hourly Rate: $20 to $22 per hour

The Mosholu Montefiore Community Center (MMCC) is a vital community resource that changes the course of lives of Bronx and Manhattan residents to a degree not otherwise possible. Each year, MMCC serves more than 40,000 preschoolers, school-age children, teens, adults, and senior citizens through support, enrichment, education, and recreation programs expanding opportunities for individuals and families to live full, productive lives.

MMCC is seeking a Part-Time Case Manager / Screener / Navigator to support individuals and families through our partnership with SOMOS Community Care. This role supports community members by identifying health-related social needs and connecting participants to MMCC programs and external community-based services addressing food insecurity, housing instability, transportation, healthcare access, and other social determinants of health.

The ideal candidate is adaptable, organized, and comfortable working independently and as part of a multidisciplinary team in community-based settings.

Duties include but are not limited to:

● Conducting health-related social needs (HRSN) screenings and participant intakes

● Assessing participant needs and identifying unmet social and support service gaps

● Developing and implementing individualized service and navigation plans

● Connecting participants to MMCC programs, SOMOS Social Care Network services, and community-based resources

● Assisting with benefits navigation and applications (e.g., SNAP, Medicaid, transportation supports)

● Providing follow-up, advocacy, and coordination to ensure continuity of services

● Maintaining accurate and timely documentation, case notes, referrals, and progress updates

● Participating in team meetings, case conferences, and required trainings
Performing outreach in community and partner settings as needed

● Other duties as assigned

Requirements:

● Bachelor’s degree preferred in Social Work, Human Services, Psychology, Public Health, Counseling, or a related field with transferable skills

● At least 1 year of experience in case management, care coordination, or social services preferred

● Familiarity with NYC benefits and community resources, including food assistance, healthcare, housing, and transportation supports

● Strong organizational, time management, and communication skills

● Ability to manage multiple tasks and maintain accurate documentation

● Proficiency in MS Office and electronic case management systems

● Ability to travel locally to community-based sites (schools, partner agencies, clinics, etc.)

● Ability to lift up to 15 lbs. for short periods of time

● Bilingual (Spanish preferred)

Job Type: Part-Time

Schedule: Flexible; may include daytime, evening, or occasional weekend hours based on program needs
