This Program is coordinated by the Rehab Department and supervisen by the Unit Manager, with Director of Nursing as backup. This may include leading guided therapeutic exercises, ROM, stretching, functional mobility and self-care activities under the guidance and supervision of a Rehab Therapist. The goal of such program is to maximize the functional capacity, prevent physical decline and promote wellbeing of Woodland Pond Residents living on the long term Skilled Nursing Unit. Serves as a liaison between Nursing Staff and the Therapy & Rehabilitation department ESSENTIAL tasks AND RESPONSIBILITIES (*CF indicates Critical Function) Works, 1:1 or in small groups of residents involved and Care Planned for a Restorative Nursing program assisting them in the completion of a therapeutic treatment regimen as developed and instructed by a member of the Therapy staff / Rehab Director or designee *CF This may include activities performed in the resident's room, bathroom, hallway or therapy gym Activities may consist of ambulation, progressive resistive exercises, ROM, stretching, self-care tasks, therapeutic diversional activities Assist in donning / doffing of corrective braces / splints Assists with toileting, personal hygiene, self-feeding and all other ADLs as these activities relate to the goals of the Restorative Nursing Program Document the Resident's participation in the Rest NSG program on a everyday basis in a Treatment Log and EMR (Sigma) so that participation can be captured on the MDS *CF Communicates with Rehab Team regularly, no less than weekly in regard to each resident's participation and progress in the program Transport Residents from their rooms to the Rehab Gym as needed for the completion of the recommended therapeutic program. May conduct small group activities as recommended by the Restorative Nursing program Inspects ambulation assistive devices for residents on program for cleanliness and state of repair Observes residents on units and intervenes for proper positioning, proper use of adaptive and protective equipment Maintains confidentiality of all resident records at all times Completes all required training and in-services All other tasks as assigned GENERAL expertise AND ABILITIES Maintains high standards for work areas, attitude and appearance Understands that honesty and ethics are essential in the performance of tasks Willing to lgain and grow Demonstrates maturity in judgment and behaviors Able to work a flexible schedule and assignment Maintains predictable and reliable attendance QUALIFICATIONS Certified by New York State as a Nursing adjunct 2-3 years significant working practice preferred Proficiency with computers and standard office software Excellent customer service and verbal communication expertise capability to multitask Demonstrated knowledge of appropriate expertise for communicating with individuals of all ages, especially the geriatric population DEGREE OF TRAVEL Occasional off-site meetings, seminars, and conferences. Frequent travel within building, possible community travel DISRUPTION TO ROUTINE Must adapt to resident status and needs as directed.
