The nurse delivers patient care by incorporating the tenets of the professional practice model of Relationship Centered Care. They assess and evaluate patient care needs and apply critical thinking skills in patient care management. The nurse integrates relevant assessment and intervention skills in the delivery of nursing care. They collaborate with the interdisciplinary healthcare team in the development, implementation, and evaluation of the plan of care. The nurse prioritizes all aspects of patient care, including teaching, rounding, coaching, and planning after hospital care. They educate patients and caregivers about their plan of care, transition of care, promotion of health, and prevention of disease. Effective communication with patients, family, and all members of the Interdisciplinary Patient Care Team is essential. The nurse manages assignments within the Care Delivery Model and demonstrates effective allocation of resources. They role model caring behaviors and facilitate a positive patient experience. The nurse considers patients' values, preferences, cultural diversity, and expressed needs in all aspects of care. Establishing and maintaining therapeutic relationships with patients and families is crucial. The nurse uses evidence-based practices to enhance understanding of patients' perceptions of care. They initiate service recovery for patients and family members who express concerns. Demonstrating empathy and active listening is integral to providing quality care. The nurse practices safe and efficient patient-centered care and identifies opportunities for improvement. They demonstrate clinical skills and knowledge crucial to quality and safety in the patient population served. Engaging in process improvement strategies based on best practices is required. Collaboration with interdisciplinary teams to implement quality improvement projects is essential. The nurse contributes to quality and safety practices and compliance measures. They implement principles of high reliability to ensure a safe environment for patients. Identifying and escalating safety hazards following institutional protocols is important. The nurse documents nursing practice accurately to support quality and performance improvement initiatives. Performing charge nurse duties and coordinating with team members for effective resource management is expected.
