Position Summary:

We are seeking a compassionate, detail-oriented Bilingual Care Coordinator fluent in English and Hebrew/Yiddish to join our team. The Care Coordinator will play a key role in supporting clients by facilitating access to services, improving communication between providers and families, and ensuring continuity of care. This position requires strong interpersonal skills, cultural sensitivity, and a deep commitment to client-centered support.

Key Responsibilities:

• Serve as the primary point of contact for English and Bengali-speaking clients and their families.

• Coordinate services and care plans across interdisciplinary teams, including medical providers, social workers, and community resources.

• Conduct client intakes, needs assessments, and follow-ups in both English and Russian

• Maintain accurate and timely documentation of client interactions and service plans.

• Advocate for clients by helping them navigate healthcare, social services, and other systems.

• Schedule and confirm appointments, assist with referrals, and provide logistical support (e.g., transportation, interpretation).

• Monitor client progress and address barriers to care or services.

• Maintain compliance with confidentiality and HIPAA regulations.

Qualifications:

• Fluency in both English and Hebrew/Yiddish (spoken and written) required.

• Bachelor’s degree in Social Work, Public Health, Psychology, or a related field preferred; equivalent experience considered.

• 1–3 years of experience in care coordination, case management, or social services strongly preferred.

• Strong interpersonal, organizational, and communication skills.

• Culturally competent and sensitive to diverse populations.

• Proficiency in using electronic health records (EHR) and case management software is a plus.

• Ability to work independently, manage multiple tasks, and remain flexible in a fast-paced environment.
