Position Title: Registered Nurse – Adult Care / Home Care RN

Location: New York, NY 10004

Employment Type: Contract (13 Weeks, High Extension Potential)

Hours: 40 hours/week

Shift: 9:00 AM – 5:00 PM

Position Overview

A leading healthcare organization in New York City is seeking an experienced Registered Nurse for a home care assignment. The RN will deliver comprehensive in-home clinical care, including patient assessment, medication administration, wound care, and education. This role requires strong independence, clinical judgment, and prior home care experience, with the ability to manage multiple patient visits daily.

Key Responsibilities

• Conduct comprehensive patient assessments and develop individualized care plans
• Provide direct in-home nursing care for pre-operative and post-operative patients
• Administer medications and monitor patient response and side effects
• Perform wound care, dressing changes, IV therapy, and catheter care as needed
• Monitor vital signs and promptly report changes in patient condition
• Operate and manage medical equipment in home settings
• Coordinate care with interdisciplinary healthcare teams and support discharge planning
• Educate patients and families on treatment plans, medications, and recovery care
• Maintain accurate and timely clinical documentation and patient records
• Follow all nursing standards, infection control protocols, and regulatory guidelines

Required Qualifications

• Minimum 2 years of recent Medical-Surgical (Med/Surg) nursing experience
• Experience in acute care hospital or post-acute care setting required
• Clinical experience must be within the last 12 months
• Prior Home Care nursing experience is required
• Ability to independently manage approximately 4–8 patient visits per day

Education & Licensure

• Associate Degree in Nursing (ADN) from an accredited institution required
• Active New York State Registered Nurse (RN) license required
• AHA Basic Life Support (BLS) certification required
• AHA ACLS required (applicable for specific assignments as needed)
• Primary Source Verification required

Preferred Skills

• Strong clinical background in Med/Surg nursing
• Experience in IV therapy, medication administration, and wound care
• Familiarity with telemetry monitoring and post-operative care
• EPIC EMR experience preferred
• Comfortable working independently in home care environments
