Travel Registered Nurse Case Management Job
- You will assess patient needs, identify barriers to timely discharge, and develop individualized, cross‑disciplinary care plans that coordinate medical, social, and community resources.
- Responsibilities include comprehensive assessments, risk stratification, discharge planning, care coordination from hospital to home or post‑acute settings, and medication reconciliation to prevent adverse events.
