Description:
- This position involves providing comprehensive care management and quality assurance for participants in a healthcare setting.
- The role requires coordinating holistic care plans sensitive to diverse backgrounds and needs.
- The position is onsite in Bronx, NY, with a contract employment type and standard weekday hours.
Requirements:
- Master's degree in Social Work from an accredited program.
- Minimum of two years of administrative experience in a management capacity in healthcare settings.
- Experience in customer service is required.
- Experience with managed long-term care insurance is beneficial.
- Minimum of one year of experience working with a frail or elderly population.
- Supervisory experience preferred.
- Must be medically cleared for communicable diseases and have up-to-date immunizations.
Qualifications:
- Current active and unrestricted license and registration in New York County.
- Bilingual skills are preferred.
- Proven ability to work independently and within a team environment.
- Strong communication and interpersonal skills.
- Ability to conduct training and competency sessions with staff.
- Knowledge of care management activities and interventions.
- Fiscally responsible for providing services based on members' needs.
