Direct Support Professional Position Overview:

Under general supervision, the Direct Support Professional (hereafter DSP) is responsible for devoting their attention to the care and development of the residents in one of the agency’s residential facilities (ICF or IRA). The DSP relates to each resident in a caring, empathic manner and is ever mindful that they are working in the residents' home. The DSP is a member of the individual's interdisciplinary team. The DSP interacts in a manner which fosters dignity and learning in a way that helps lead the individuals to greater independence. Helps create a homelike atmosphere for residents.

• Provides for the physical care needs of individuals in accordance with the individual's care plan and program.
• Transports and/or escorts residents in agency vehicles to and from work, shopping; accompanies residents to programs and appointments.
• Reports immediately to supervisor any allegation of abuse or mistreatment.

High School Diploma or Equivalency Required

Pay Rate: $17.86/Hour

Case Manager Position Overview:

Under general supervision, is responsible for the development and implementation of the Individualized Service Plans for individuals and their advocates in accordance with the regulations and requirements of the applicable governmental and other regulatory agencies within a residential setting. Provides case management services to assigned individuals and determines individual aptitudes, interests, and preferences. Identifies appropriate service providers and links individuals with providers. Coordinates services and monitors individual’s satisfaction with the services.

Bachelor's Degree Required

OPWDD Experience Required

Pay Rate: From $25.50/Hour
