• Sunday (7:00 AM–5:00 PM)
• Tuesday (7:00 AM–5:00 PM)
• Wednesday (7:00 AM–5:00 PM)

✅ Qualifications

✔️ Associate’s or Bachelor’s Degree in Nursing
✔️ Active New York State RN license
✔️ Minimum of 6 months clinical experience
✔️ Experience with home care assessments and OASIS preferred
✔️ Strong documentation, communication, and organizational skills

✅ Key Responsibilities

✔️ Conduct home care assessments and re-assessments using OASIS
✔️ Collaborate with physicians, family, and care teams to develop care plans
✔️ Provide skilled nursing care including treatments and preventive procedures
✔️ Observe, report, and document changes in patient condition
✔️ Educate patients and families on care and wellness
✔️ Maintain accurate documentation per facility policies
✔️ Supervise and orient home health aide staff
✔️ Uphold infection control, safety, and client rights standards
