Apicha Community Health Center aims to improve community health by providing access to comprehensive primary care, preventive health services, mental health, and supportive services. Position Summary
The Community Health Navigator will help recruit patients for Apicha CHC’s Jackson Heights health center, particularly outreaching to Bangladeshi and/or Filipino communities in Western Queens. This role involves working closely with staff from different departments to make sure patients are smoothly connected to care. This role will conduct outreach to Asian Pacific Islanders communities in Western Queens and promote Apicha CHC’s services. The role also includes assisting with administrative tasks.
The major duties of the position include, but are not limited to:
- Outreach - Recruit patients from priority populations
- Education - Educate our priority communities on various health and mental health topics.
- Client Service - Help individuals and families access health services in a culturally and linguistically appropriate way.
- Compliance - Follow policies and procedures for data entry, reporting, and meeting deadlines.
Qualifications:
- High school diploma/GED with 2 years of work experience, or some college with 1 year of work experience.
- Ability to communicate in English; fluency in an API language is a plus.
- Ability to work outside of traditional 9-5 office hours including evenings and weekends.
- Ability to reach out and recruit program participants from the target community.
- Strong team player with the ability to work independently.
- Reliable, punctual, and adaptable.
- High energy with excellent interpersonal and communication skills.
- Comfortable working with a diverse population in a fast-paced environment.
- Ability to manage multiple tasks and work well within the organization.
- Comfortable using public transportation.
- Proficiency with Microsoft Office (Word, Excel, PowerPoint).
