The Care Manager, Telephonic Nurse 2, in a telephonic environment, assesses and evaluates members' needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. The Care Manager, Telephonic Nurse 2 work assignments are varied and frequently require interpretation and independent determination of the appropriate courses of action.
The Care Manager, Telephonic Nurse 2 employs a variety of strategies, approaches and techniques to manage a member's physical, environmental and psycho-social health issues. You will follow our tiniest members from the beginning of their NICU stays until 30 days post-discharge. You will identify and utilize appropriate healthcare resources most consistent with the member's needs and by providing guidance along the healthcare continuum.
Identifies and resolves barriers that hinder effective care. Ensures patient is progressing towards desired outcomes by continuously monitoring patient care through assessments and/or evaluations. May create member care plans. Understands department, segment, and organizational strategy and operating objectives, including their linkages to related areas. Makes decisions regarding own work methods, occasionally in ambiguous situations, and requires minimal direction and receives guidance where needed. Follows established guidelines/procedures.

Required Qualifications

Licensed Registered Nurse (RN) in the (appropriate state) with no disciplinary action
-
Active Compact RN license with the ability to apply for additional licensure without restrictions if needed

2 years or more of case management experience
-
Comprehensive knowledge of Microsoft Office applications including Word, Excel, and Outlook

Ability to work independently under general instructions and with a team
-
Team player
-
Strong organizational skills and experience multi-tasking
Preferred Qualifications
-
BSN or Bachelor's degree in a related field
-
Registered Nurse (RN) with 3 years or more of NICU level 3 experience
-
Experience with discharge planning and patient education for NICU, pediatrics, and adult acute care
-
Managed care experience
-
Certified Case Manager (CCM)
-
Bilingual is a plus - preferably Spanish

Humana will provide Home or Hybrid Home/Office employees with telephone equipment appropriate to meet the business requirements for their position/job.

Work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information.

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
$71,100 - $97,800 per year
This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
