Family Support for Student Success Specialist - Remote | WFH
As a Family Support Specialist, you will play a vital role in bridging the gap between home and school by providing support to families, teachers, and other community partners. Your efforts will help remove barriers to student success, ensure the safety and welfare of children, and promote a supportive environment for academic achievement. Key Responsibilities:
Family Support and Home Visits: Conduct bi-weekly home visits with up to twelve families facing challenges.
Develop and implement goal-driven service plans and provide ongoing case management.
Work collaboratively to connect families with necessary resources and services.
Assess safety and risk factors for children during each contact.
Coordinate transportation and other services as needed to ensure comprehensive support. School Collaboration: Act as a key member of the School Intervention Team, assisting with assessments and interventions.
Build and maintain strong relationships with school staff, service providers, and community organizations.
Facilitate communication between families, teachers, and support teams to create a unified plan for student success. Professional Conduct: Provide culturally competent and strength-based services to families and students.
Ensure compliance with all organizational policies, including those related to documentation and reporting.
Complete required training and assessments as mandated by state and local regulations. Desired Skills and Qualities:

A Bachelor's degree or equivalent experience.
Experience working with families or in a school setting is a plus.
Ability to complete required background checks, including criminal and DMV screenings.
Valid New York State Driver's License and reliable transportation.
Additional Information:
This role involves regular visits to assigned schools (three days a week) and a remote location (two days a week).
