Case Manager - 5030

Across our 60+ locations, we provide programs in a variety of service areas, including workforce development, children, youth, families, seniors and homeless prevention.
The Living Room-Safe Haven is a 50 bed, low-barrier shelter operated by BronxWorks for chronically street-homeless single adults. Located in the Hunts Points section of the Bronx, The Living Room-Safe Haven follows a Housing First model and provides on-site services including case management, psychiatric and medical care, substance use counseling, meals, showers, and housing placement support. Referrals are made exclusively through NYC’s Street Homeless Outreach Teams. The program focuses on building trust and engagement while supporting clients in transitioning into stable, long-term housing.
RESPONSIBILITIES
- Independently manage a caseload.
- Conduct comprehensive intake assessments.
- Conduct regular field visits to engage and build rapport with chronically homeless individuals.
- Work with clients to develop goals and service plans.
- Provide referrals and direct assistance to support clients applying and/or attending appointments for mental health services, medical care, substance treatment, vital documents, benefits, Legal/immigration services, and permanent housing.
- Document all interactions with or on behalf of clients, monitor and document client progress toward service plan goals.
- Maintain familiarity with social services and resources available both on-site and in the community.
- Coordinate client care with other service providers; attend regular case conferences with clients and other service providers, as required.
- Attend regular supervision sessions with direct supervisor.
- Drive agency vehicle to and from client locations and appointments.
- Perform additional duties as assigned.
QUALIFICATIONS
- Bachelor’s degree in social work or a related field of study
- Candidate must have a valid driver’s license which has been held for at least 1 year; in the last 3 years, candidate must have obtained no more than 3 license violation points
- One-year of case management or related work experience preferred.
- Ability to engage and build rapport with populations with high support needs.
- Strong oral and written communication skills
- Detail-oriented and resourceful with strong time management and organizational skills
- Proficiency in Microsoft Office suite and other standard business technology
- Bilingual/multilingual preferred but not required.
- Valid driver’s license is required for this position.
PHYSICAL REQUIREMENTS
- Excellent map reading skills and ability to navigate public transportation system.
- Excellent organizational and interpersonal skills.
- Strong grammar skills.
- Ability to use a computer for prolonged periods.
- Ability to occasionally lift and/or move up to 10 pounds.
- Ability to stand, walk, or sit for long periods of time.
- Ability to climb five flights of stairs.
- Ability to bend and retrieve objects and/or documents.
- Ability to travel in the boroughs of New York City and its adjacent counties via public transportation.
- Ability to travel for a minimum of 1 mile within a reasonable amount of time and in all types of weather, including inclement weather
