Description:
- Seeking a Bi-lingual Nurse Practitioner for an Adult Day Treatment Center in Brooklyn and Queens, NY.
- This is a contract position with a schedule of 08:00AM-05:00PM, three days in Brooklyn and two days in Queens.
- The role involves collaboration with primary care physicians and interdisciplinary teams to support participant care, focusing on functional, clinical, and psychosocial needs.
Requirements:
- Licensed Nurse Practitioner in NY.
- Licensed Registered Nurse in NY.
- DEA Certification.
- AANP or ANCC Board Certification.
- Minimum of two years in clinical nursing practice.
- Experience working with frail or elderly populations.
- Bi-lingual in Spanish with effective communication skills.
Qualifications:
- Graduated from an accredited nurse practitioner program.
- Two years of experience in home care, care management, or related field.
- Proficient in Microsoft Office.
- High-level analytical and research skills.
- Ability to manage multiple projects and meet deadlines.
- Medicaid and Medicare Provider IDs in good standing.
- Medically cleared for communicable diseases with up-to-date immunizations.
