Chronic Care Management-LPN - Remote

Are you a dedicated and compassionate nurse looking to make a meaningful impact in patient care—all from the comfort of your home? We are seeking an experienced LVN, LPN, or RN to provide telephonic care management services for patients across our health system. In this role, you'll collaborate closely with providers to support patients with chronic conditions, offer education, and help coordinate their care.

About the Role:
As a Care Management Nurse, you will serve as a key point of contact between patients, families, providers, and other healthcare professionals. Using your clinical expertise, you will assess patient needs, identify gaps in care, and provide essential education to promote self-management and improved health outcomes.

This role is fully remote and involves coordinating care via phone, ensuring patients receive the right resources and support based on their provider's recommendations.

Key Responsibilities:
Work collaboratively with patients, families, and providers to develop personalized care plans and goals.
Provide outreach and engagement to ensure patients stay on track with their treatment plans.
Offer patient education to support self-management of chronic conditions.
Identify barriers to care and assist in addressing them.
Coordinate appointments, referrals, and community resources (e.g., home health, durable medical equipment).
Encourage and guide patients toward healthy lifestyle changes.
Maintain an assigned caseload of patients, monitoring their health and progress.
Conduct outbound calls to assess health status and provide guidance under provider direction.
Document patient interactions accurately in the Electronic Health Record (EHR).
Keep the care team updated on new patient developments.
Participate in ongoing training and professional development.
Perform on-call duties as assigned.
What You Bring:
Active and unrestricted NYS LVN, LPN, or RN license.
At least 3 years of experience in Care Management, Case Coordination, home care, inpatient settings, physician practices, or in-home case management.
Strong knowledge of chronic disease management, including chronic kidney disease, diabetes, COPD, and heart failure.
Ability to problem-solve and recommend solutions for care barriers.
Experience working with Electronic Health Records (EHR).
Ability to type 60 words per minute.
Bilingual in Spanish is a plus!
