Job Title: Registered Nurse (RN) – Adult Care

Location: Bronx, NY

Duration: 11/03/2025 – 02/03/2026

Shift: 8 Hours/Day | 5 Days/Week | 40 Hours/Week

Schedule: 9:00 AM – 5:00 PM

Job Summary:

The Registered Nurse (RN) – Adult Care provides direct at-home nursing care to adult patients ranging from 18 to 100 years old within the Bronx borough. The RN travels to patient homes to assess, monitor, and manage patient health needs, delivering high-quality, compassionate care while maintaining accurate documentation and adhering to clinical standards.

Responsibilities:

• Provide skilled nursing care in patient homes, including assessments, medication administration, and treatment planning.

• Travel to multiple patient homes per day (approximately 5–8 cases).

• Develop and implement individualized care plans based on patient needs.

• Monitor patient health status and communicate updates to physicians as necessary.

• Educate patients and families on disease management, medication use, and health maintenance.

• Maintain accurate and timely documentation of all patient interactions in compliance with clinical standards.

• Collaborate with the healthcare team to ensure coordinated and effective patient care.

• Adhere to infection control, safety, and patient privacy protocols.

Required Skills & Experience:

• Minimum 2 years of adult care or medical-surgical nursing experience.

• Strong assessment, communication, and clinical decision-making skills.

• Ability to work independently in a home care setting.

• Comfortable traveling within the Bronx borough to provide care.

Licenses & Certifications:

• Active and unrestricted Registered Nurse (RN) license (NY).

• Current BLS certification (AHA) required.
