Position Overview:

Schedule & Location:

• Full-time position
• Brooklyn, NY (Field-based role)

Qualifications:

• NY RN license (active).
• Min. 6 months clinical experience.
• Must drive & have valid driver’s license.
• Experience with HCHB software preferred.

Key Responsibilities:

• Conduct home care assessments & create care plans.
• Deliver skilled nursing (wound care, injections, medication management, etc.).
• Document visits in HCHB software.
• Coordinate care across MDs, patients & family.
• Ensure timely recertifications & discharge planning.
