We are a reputable healthcare organization located in New Windsor, NY, seeking a dedicated and compassionate Hospice Case Manager Nurse to join our team. As an Hospice Case Manager Nurse, you will play a crucial function in providing comprehensive and compassionate care to our patients. You will be responsible for conducting assessments, developing care plans, reviewing medications and treatments with caregivers, attending meetings, educating patients and caregivers, and supervising assigned personnel. Your skills in case management and palliative care will be instrumental in ensuring the highest level of quality care for our patients.
Hospice Case Manager Nurse Qualifications and practice:
»Current NYS RN license in good standing
»Previous practice in case management, preferably in palliative care
»Excellent communication skills, both verbal and written
»Strong organizational skills and attention to detail
» capability to effectively prioritize and manage multiple tasks
»Compassionate and patient-centered approach to care
»Have a valid NYS driver's license
Hospice Case Manager Nurse Benefits:
»Pto accrued located on hours worked.
»Approximately 3.5 weeks
»Sick accrued located on hours worked
»Approximately 1.5 weeks
»Tuition reimbursement up to $3,000 per year after one year of employment
» 2% employer 403b contribution plus 3% company match after one year of employment
»Medical, Dental, and Vision insurance
»Assistance with deductible (HRA) (75%)
Hospice Case Manager Nurse Key Responsibilities:
»Conduct assessments to evaluate patients' physical, emotional, and social needs
»Develop individualized care plans located on the assessment findings
»Review medications and treatments with caregivers to ensure adherence and knowledge
»Attend weekly meetings to discuss patient status and collaborate with the interdisciplinary team
»Communicate changes in patient status to family members and clients
»Educate patients and caregivers on disease management, self-care, and available resources
»Provide counseling and emotional support to patients and caregivers
»Supervise personnel assigned to patients, ensuring proper training and adherence to care plans
»Responsible for admissions and ensuring the performance improvement program is implemented effectively
»Fulfill assigned case assignments in a timely and efficient manner
»Possess effective communication, organizational, and writing skills
»Familiarity with charting, care plans, and performance improvement programs
This is a direct hire position, offering a full-time schedule, Monday-Friday, from 9:00 AM to 5:00 PM. Compensation will be competitive and commensurate with practice.
