Medical Director (IM / FM / Geriatrics) – Post Acute Care - Syracuse, NY

Compensation & Benefits:
- $324,994 – $365,211+ with uncapped earning potential

Responsibilities:
- Comprehensive patient assessments and ongoing care management
- Manage acute and chronic conditions
- Coordinate care with interdisciplinary teams
- Maintain timely documentation and quality standards
Qualifications:
- BC/BE in Internal Medicine, Family Medicine, or Geriatrics
- New York license (or ability to obtain)
