Position Summary
CLOTH seeks a mission-driven LCSW to provide assessment, counseling, and case management for clients participating in a 5-year stroke study—conducted in partnership with New York-Presbyterian Hospital and Columbia University Irving Medical Center and funded by the National Institutes of Health (NIH)—as well as clients served through CLOTH’s social services programs. The LCSW is expected to deliver evidence-based, trauma informed care; coordinates services; and collaborates closely with a multidisciplinary research and program team.
Core Responsibilities
- Conduct comprehensive psychosocial assessments for study participants and other CLOTH clients.
- Provide therapy/psychotherapy, evidence-based interventions, and appropriate referrals.
- Lead and oversee case management, including benefits navigation, care coordination, and warm handoffs.
- Document services and outcomes in Apricot (CLOTH’s data system) and the study’s tracking platform; maintain timely, accurate notes that meet research and compliance standards.
- Share case/status updates and clinical insights with the study team and CLOTH leadership.
- Identify and document opportunities to improve service delivery based on client interactions and outcomes.
- Supervise and mentor other social CLOTH social workers.
Day-to-Day Duties
- Offer empathetic emotional support, guidance, and resource access to address client challenges.
- Perform proactive follow-up and care coordination; track client progress and barriers. • Collaborate with research partners to ensure protocol adherence and participant retention.
- Participate in team meetings, case conferences, and quality-improvement activities.
- Maintain confidentiality and uphold ethical standards in accordance with HIPAA and professional guidelines.
Required Qualifications
- Fluent in Spanish and English (spoken and written).
- MSW and active NY State LCSW license.
- 3–4 years post-licensure experience serving low-income and diverse communities.
- Effective working with children, youth, adults, and older adults.
- Proficiency in Microsoft Office (Word, Excel, PowerPoint) and comfort with data systems (e.g., Apricot).
- Excellent presentation, oral/written communication, and problem-solving skills.
- Completed Supervisor in Field course; qualified to supervise student interns.
Preferred Skills & Experience
- Experience with stroke survivors, individuals with physical disabilities, or neurorehabilitation.
- Prior work within a clinical research or medical study environment and adherence to protocols.
- Strength in coaching/supervising interns or early-career social workers.
- Ability to lead through crises, prioritize competing demands, and work independently within a fast paced, team-oriented setting.
- Deep knowledge of NYC health and social service systems (government, nonprofit, and low/no-cost resources).
- High levels of empathy, patience, cultural responsiveness, and a strengths-based, hope-centered approach.
Schedule & Work Location
- Full-time, onsite at 2185 Amsterdam Ave, NYC with regular citywide travel for client work, partner meetings, and community visits.
