Join The Doe Fund on our mission to break the cycles of homelessness, addiction, and recidivism. Our flagship Ready, Willing & Able (RWA) program provides paid transitional work experience, transitional housing, advanced occupational training, and comprehensive supportive services to individuals experiencing homelessness. The Doe Fund is also one of New York City’s largest nonprofit developers of permanent, affordable, and supportive housing, with more than 1,900 units in operation or pre-development. The Doe Fund has an annual operating budget of $80 million and a full-time staff of nearly 500, a significant portion of whom are graduates of our programs.

We are seeking a Case Manager for one of The Doe Fund's permanent supportive housing for homeless individuals living with a serious mental illness and/or history of substance use. The housing program is designed to provide long-term residential care and twenty-four-hour support services in a community environment that maximizes residents' independence while ensuring their physical and emotional security and well-being.

The Case Manager will play a crucial role in supporting the residents referred by HASA. Responsibilities include providing comprehensive case management services and counseling tailored to the needs of these individuals. The successful candidate for this position should demonstrate strong clinical skills and prior experience working with individuals diagnosed with co-occurring disorders and/or trauma.

RESPONSIBILITIES:

  • Maintain a caseload of individuals diagnosed with serious mental illness and/or substance use disorder and provide supportive case management services to them
  • Develop and facilitate therapeutic groups for residents
  • Maintain resident records in compliance with agency and funding standards
  • Conduct and review assessments
  • Develop and implement individual Service Plans and assist residents with achieving their goals geared towards self-sufficiency
  • Monitor and ensure residents are linked to appropriate treatment and services
  • Provide counseling and crisis intervention as necessary
  • Assist residents in securing and maintaining benefits
  • Maintain a liaison with community services, including hospitals, substance abuse treatment programs, HRA, and mental health programs
  • Monitor and assist residents with rent payments and budgeting
  • Conduct regular unit inspections
  • Educate residents about approaches to mental health and substance use
  • Compile resources on mental health information and treatment strategies
  • Attend ongoing training to enhance the services provided to the residents
  • Assist with the completion of reports as needed
  • Other duties as assigned

QUALIFICATIONS:

  • A Bachelor’s degree in Psychology, Mental Health Counseling, Social Work, or a relevant field . Master’s degree preferred.
  • At least 3 years of experience working with homeless individuals diagnosed with serious mental illness and/or substance abuse disorder and/or in trauma settings
  • Expertise in Evidence-Based Practice, including Critical Time Intervention, Motivational Interviewing, and Harm Reduction techniques
  • Excellent clinical and case management skills, including knowledge of benefits in the NYC Metro area
  • Sensitivity to community issues and needs
  • Outstanding interpersonal and communication skills, both oral and written
  • Strong ethical standards, confidentiality, and personal integrity
  • Computer proficiency (i.e., email, Microsoft Word, Excel, and PowerPoint)
