Program Overview

The Van Dyke Cornerstone Case Management Expansion is an enhancement to CAMBA’s DYCD-funded Cornerstone Community Center at Van Dyke Houses that brings an intensive, case-management approach to young adult participants ages 18–21. Using a strength-based, youth-centered, and trauma-informed lens, the program team engages high-risk young adults, develops Individual Service Plans (ISPs) addressing education, employment, health, and life skills, and acts as “expediters” connecting participants to City agencies and community providers for health, mental health, employment training, housing, food, and other supports.

What Case Manager Does:

  • Practice Universal Precautions/Standard Protocol & Procedures. 
  • Comply with all Federal, State, City, and CAMBA security and privacy policies to protect the confidentiality of individual health information. 
  • Ensure the safety and security of all participants by following program protocols and procedures, including supervision, accident and incident reporting. 
  • Initiate contact and establish trust with identified high-risk participants ages 18–21 at the Cornerstone Community Center. 
  • Holistically assess each participant’s strengths and needs, and develop an Individual Service Plan (ISP) that includes, at a minimum, goals for education, employment, health, and life skills. 
  • Meet individually with each assigned participant on a weekly basis to review progress, adjust goals, problem-solve barriers, and move participants toward completion of their ISP goals. 
  • Identify participants who have experienced or are experiencing trauma and, in coordination with the clinical supervisor, establish and support weekly Healing Circles for them and for high-risk groups. 
  • Make, track, and follow up on referrals to City agencies, community-based organizations, and other providers for health, mental health, employment and training, housing, food, benefits, legal, and educational services. 
  • Serve as an “expediter” for participants by accompanying, advocating, and coordinating with partner providers to ensure referrals result in actual connection to services. 
  • Work closely with the Youth Navigator on the implementation of ISPs, including outreach, re-engagement, and continuous support for participants. 
  • Participate in clinical supervision with a Licensed Master Social Worker (LMSW) virtually at least twice per month, and present cases for review, consultation, and guidance. 
  • Provide crisis intervention and de-escalation support to participants, and follow established protocols for mandated reporting, safety planning, and escalation to the clinical supervisor. 
  • Work with participants to foster positive relationships with peers and adults, and support the development of life skills, self-advocacy, and healthy decision-making. 
  • Support participants’ family needs, and provide or refer families to additional resources and services. 
  • Maintain complete, accurate, timely, and confidential case records, including intake and assessment documents, ISPs, weekly progress notes, referral and follow-up logs, and case closings, in accordance with CAMBA and funder requirements. 
  • Enter and maintain participant data in required databases and prepare case-related documentation for monthly reports, funder reporting, and site visits. 
  • Participate in case conferences with the clinical supervisor, Youth Navigator, Peer Support Leaders, and partner providers to coordinate care and avoid duplication of services. 
  • Serve as a supportive resource to other on-site staff at the Cornerstone Community Center on participant needs and service connections. 
  • Attend required staff meetings, supervision, trainings, partner meetings, and program events. 
  • Maintain appropriate professional boundaries with participants and follow all CAMBA, DYCD, and site-based policies, procedures, confidentiality standards, and safety protocols. 
  • Tasks may be modified, expanded, and/or assigned over time to meet program needs. 

Qualifications:

Minimum Education Required:

  • Associates degree in human services, psychology, counseling, or a related field from an accredited college,  

Minimum Experience and Skills Required:

  • Minimum of two (2) years of case management experience, or experience in community work or community-centered activities related to the duties of this position. 
  • Experience working with high-risk, justice-involved, or system-impacted young adults preferred. 
  • Experience conducting assessments and developing and implementing service or treatment plans. 
  • Strong communication, documentation, and organizational skills. 
  • Proficiency with Microsoft Office and case management database systems. 


Pre-employment Requirements:

  • If off-site or school-based , may require: Dept. of Health, Dept. of Education fingerprint clearance; doctor's note certifying good health; NYS Division of Criminal Justice Services Sex Offender Registry clearance  

Compensation : $53,000 annually 

Status:  Full-time
