Job Overview:
HealthYes is seeking a compassionate and experienced Family Nurse Practitioner in the Great Neck area, NY. This position focuses on delivering patient-centered care, managing acute and chronic conditions, and promoting health and wellness within a collaborative healthcare environment.
Key Responsibilities:
- Conduct comprehensive patient assessments, including health histories and physical examinations.
- Diagnose and manage acute and chronic illnesses, developing personalized treatment plans.
- Address preventative care needs and close care gaps for assigned patients.
- Order and interpret diagnostic tests and lab results; manage follow-ups.
- Collaborate with healthcare professionals and community resources to ensure holistic care.
- Participate in quality improvement initiatives and departmental committees.
- Provide supervision and training to medical staff, including students and residents.
Qualifications:
- Master of Nursing from an accredited institution.
- Active New York State Family Nurse Practitioner license.
- Spanish or Mandarin speaker preferred.
Experience Requirements:
- 2-3 years of experience as a practicing Family Nurse Practitioner (FNP) preferred.
- Familiarity with medical devices (e.g., glucometers, blood pressure monitors).
- Experience with community resource facilitation and referrals is advantageous.
