Physical Therapist Job Details

• Job Title: Locum Tenens Physical Therapist
• Employment Type: Travel Contract (Locum Tenens)
• Location: Webster, NY 14580 (Finger Lakes Region, New York healthcare jobs)
• Setting: Hospital-Based Home Health
• Schedule: Monday - Friday, 8:00am - 4:30pm; weekends as needed
• Hours per Week: 40
• Shift Duration: 8-hour shifts
• Start Date: September 15, 2025
• End Date: December 13, 2026
• Contract Length: 65 weeks
• Mileage: Reimbursed
• Estimated Weekly Salary: $1870 - $1920

Physical Therapist Qualifications

• B.S., M.S., or D.P.T. degree in Physical Therapy (required for PT jobs)
• Active New York State Physical Therapy License
• BLS certification (AHA only)
• Minimum 1 year of experience as a Physical Therapist in home health or long-term care
• Recent home health experience within the last year
• Strong communication and documentation skills
• Ability to work with diverse patient populations
• Physical ability to lift and assist patients as needed

Physical Therapist Responsibilities

• Deliver direct patient care and case management in a collaborative healthcare team
• Administer comprehensive physical therapy modalities in home health settings
• Collaborate with physicians to evaluate patient function and develop individualized care plans
• Prepare and update clinical and progress notes; revise care plans as necessary
• Educate patients, families, and team members on disease management, self-care, and prevention
• Implement home therapy programs, including gait training, transfers, ADLs, and equipment use
• Supervise Physical Therapist Assistants (PTAs) and Home Health Aides to ensure quality care
• Participate in interdisciplinary team meetings and patient conferences
• Maintain accurate clinical records in compliance with New York state and federal regulations
• Promote agency mission and values through quality patient care and positive relationships
• Engage in continuous improvement and professional development activities
