Sun River Health is a Federally Qualified, Non-Profit Health Center serving communities in Suffolk, Rockland, Orange, Dutchess, Ulster, Sullivan, Columbia and Westchester County. Sun River Health is currently seeking a full-time Peer Adherence Worker to join our team in the Peekskill, NY.The position is a full-time position at our Bronx, Thrive Center providing adherence education and emotional support to HIV positive patients in our HIV primary care and case management programs at Sun River Health. The Peer Adherence Worker works with the HIV Specialist and support staff to reduce barriers to healthcare for patients, assess and educate patients on the importance of medical adherence and harm reduction, escort patients to appointments as needed, and complete referrals and follow up contact with patients to encourage retention in care. The peer shares life practice to provide strategies to patients to assist in coping with living with HIV.
Essential tasks and responsibilities of the Peer Adherence Worker :
Conducts adherence assessments with individuals living with HIV to identify and reduce barriers to care
Refer patients to community resources
Implements evidence-located strategies
Provides support and follow-up on patient needs
Provides harm reduction and adherence education
Qualifications required High School diploma/GED, availability to share HIV+ status life practices as a teaching method, expertise of HIV disease and treatment, demonstrate good verbal and writing expertise, initiative, self direction, enthusiasm, cultural sensitivity, capability to interface with community agencies. Bilingual Spanish/English speaking preferred.
Hourly Rate: $16.00 - $17.50 / hour
