At Sun River Health, we are dedicated to providing high-quality primary, preventive, and behavioral health services to all individuals, regardless of their insurance status and ability to pay. We are currently looking for a full-time Licensed Practical Nurse to enhance our care team.

About the Role

As a Licensed Practical Nurse, you will play a vital role in providing nursing care to patients. You will assist healthcare providers and work within our outpatient care settings, adhering to the policies and objectives of the Center.

Key Responsibilities:

• Conduct patient interviews and maintain accurate medical records, including nursing observations, health history, vital signs, and medication usage.
• Perform in-house laboratory tests, collect specimens, and manage quality control in accordance with established protocols.
• Ensure all tasks and logs are completed and updated regularly.
• Initiate follow-up actions as necessary, guided by the healthcare provider.
• Provide health education as directed by the physician or based on patient inquiries, and refer patients to Registered Nurses when appropriate.
• Gather patient data under the supervision of a Registered Nurse or provider, ensuring compliance with protocols.

Qualifications:

• Education: High school diploma/GED required; completion of a Vocational/Technical Nursing program preferred.
• Experience: 0-1 year of related experience required; 2-4 years preferred.
• Licensure: Valid LPN license in New York State and CPR/Infection Control Certification.
