Director, Case Management - MSN

We are seeking a highly motivated and experienced Director of Case Management to oversee and enhance the effectiveness of our case management department. This leader will be responsible for budget management, strategic planning, team supervision, and ensuring high-quality patient care coordination and discharge planning in compliance with hospital policies and regulatory standards.

Key Responsibilities:

Direct and coordinate all aspects of the Case Management department, including utilization review, care coordination, and discharge planning.

Oversee and manage the departmental budget and resource allocation.

Plan and implement educational activities to support the ongoing professional development of case management staff.

Ensure effective communication and collaboration between Case Management, clinical teams, and external agencies to optimize patient outcomes.

Maintain compliance with hospital policies and standards of care set by federal, state, and accrediting agencies (e.g., CMS, Joint Commission).

Monitor and evaluate the performance of case management processes and staff to ensure continuous quality improvement.

Develop and implement strategic initiatives aligned with hospital goals to enhance care coordination and patient transitions.

Qualifications:

Master’s Degree in Nursing (MSN) – required

Current New York State Registered Nurse (RN) License – required

Minimum of three (3) years of progressive leadership experience in Case Management

Strong knowledge of utilization review, discharge planning, care coordination, and regulatory compliance

Proven ability to lead multidisciplinary teams and implement effective performance improvement initiatives
