We serve patients across a large area, and this specific opportunity is based in the Placerville area of Alameda County, CA.

What You'll Do:

Provide Home-Based Care: Partner with the Case Manager to provide skilled occupational therapy services to individuals in their homes.
Assess & Treat: Evaluate patient needs and develop individualized treatment plans to help them live safely and productively.
Areas of Focus: Address areas such as home safety, medication management, wheelchair seating and positioning, pain management, falls prevention, chronic disease management, dementia care, and caregiver training.
Educate & Advocate: Serve as an educator, patient advocate, and valued member of the interdisciplinary care team.

What You'll Bring:

Licensure: Current unrestricted CA Occupational Therapist License.
Certification: Current BLS/CPR Certification.
Driver's License: Valid CA Driver's License.
Local Knowledge: Knowledge of local community resources.
Understanding of Human Behavior: Knowledge of individual development and human behavior related to illness/injury, and the influence of culture on healthcare.
Regulatory Knowledge: Knowledge of state and federal homecare regulatory guidelines (preferred).
Experience: Home health care experience (highly preferred), experience with high acuity patients (a plus), experience working with an interdisciplinary team (highly desired), and case management fundamentals (highly desired).
Personal Qualities: Self-directed, empathetic, critical thinker, outstanding interpersonal skills, leadership skills, and organized.
Computer Skills Assessment:

Prior to an interview, candidates will complete a computer literacy and competency assessment to ensure preparedness for electronic charting and our communication systems.

Compensation:

$120,000 - $130,000 annually. Starting pay is based on relevant experience. Generous bonus incentives, comprehensive health benefits, and 401(k) with matching are offered.
