Compensation: $40/hour or $125 per visit

Affirmed Home Care is growing and seeking compassionate, skilled Licensed Practical Nurses (LPNs) to join our home care team. What You’ll Do
As a Per Diem LPN, you will provide skilled nursing care to clients in their homes throughout Westchester County. You’ll work independently while still having the full support of a responsive clinical team. Assignments include 12-hour day shifts, overnight shifts, and skilled nursing visits.

What We’re Looking For
• Active New York State LPN license and current BLS certification
• Minimum 1 year of recent LPN experience in an acute care setting (home care experience preferred)
• Eligibility to work in the United States
• Recent physical exam (within 1 year)
• Current PPD or QuantiFERON (within 1 year) or chest X-ray (within 5 years)
• MMR immunizations within the last 10 years
• Valid driver’s license preferred
