QUALIFICATIONS :

• Candidate must possess a Bachelor's (Masters preferred) of Science degree in Occupational Therapy from a NYS-registered or AOTA-accredited program.

• Candidate must possess a valid New York State Occupational Therapist (OT) License.

• Candidate must possess fingerprint clearance from New York State Education Department.

DUTIES AND RESPONSIBILITIES :

• Reviews all available documentation related to the individual prior to beginning treatment; including the IEP/IFSP, script and prior evaluations if available

• Develops an appropriate treatment plan for each individual

• Performs treatments according to IFSP/IEP and accepted professional standards and practices

• Conducts ongoing assessment of individual during each session, and modifies goals/outcomes/treatment plan accordingly

• Performs appropriate parent/caregiver education and develops home programs as necessary

• Works as part of an inter-disciplinary team, utilizing communication with all team members to enhance the treatment program of the individual

• Maintains an accurate schedule

• Submits all required documentation of sessions according to established procedures

• Performs evaluations as assigned and when necessary

• Communicates with families on a regular basis

• Keeps abreast of legal responsibilities, professional ethics, standards of practice and agency rules, policies and regulations.

• Maintains client confidentiality with HIPPA/FERPA and agency guidelines

• Maintains professional appearance, follows agency dress code

• Attends department meetings and any other meetings required by your supervisor

• Performs other duties and assumes other responsibilities as assigned by their direct supervisor
