Community Coach – 35 Hrs/wk (LO)

Shift : Monday – Friday, 8:30A – 4:30P

Salary : $23.00/hr

Qualifications:

• Associate’s Degree
• Clean and valid NYS Driver’s License required
• HHA experience preferred
• Day Habilitation experience, 1 year of supervisory experience preferred, but not required

Key Responsibilities include, but not limited to:

• Assist consumers in planning appropriate community integration activities consistent with accessibility of community site.
• Promote activities to include consumer choice.
• Facilitate integration to each site, encourage consumer participation in dialogue, and solicit feedback consistent with learning module to enhance learning and awareness.
• Assist consumers with all ADL needs including, but not limited to, feeding, transferring, lifting, toileting, dressing (coats), etc., as per consumer's protocols.
• Ensure needs of consumers are met including but not limited to their safety and well-being.
• Verify consumer appointments and schedules are adhered to.
• Drive consumers to and from program in AM and/or PM.

Physical Demands:

• Must be able to bend, lift and push more than 100 pounds.
• Must be able to stand and walk for extended periods of time.
• Provide hand-over-hand assistance in classroom when necessary.
