Temporary Dietitian

We are seeking a Registered Dietitian (RDN, CDN) to join a dynamic PACE (Program of All-Inclusive Care for the Elderly) team in Flushing, NY. The Dietitian plays a vital role in developing and implementing individualized nutrition care plans for elderly participants in both community-based and center-based settings. This is an excellent opportunity to support comprehensive, person-centered care for a diverse and medically complex geriatric population.

Compensation:$43.50 – 51.00 Per Hour

Key Responsibilities:

Conduct initial face-to-face and telehealth assessments to evaluate participants' nutritional status using clinical judgment, guidelines, and participant interviews

Reassess participants as required, and modify care plans accordingly

Calculate therapeutic diets based on individual diagnoses, preferences, allergies, and comorbidities

Collaborate with physicians, nurse practitioners, and the interdisciplinary team to develop and revise nutrition treatment plans

Maintain accurate and up-to-date documentation of diet orders, restrictions, assessments, and annual reviews

Provide individual and group nutrition counseling and education to participants, caregivers, and staff

Recommend nutrition-related clinical orders for approval by medical providers

Monitor participants’ response to nutritional interventions and ensure care is integrated into the overall interdisciplinary team (IDT) plan

Actively participate in care planning meetings and contribute to improving participants' overall health, wellness, and independence

Schedule:

Monday–Friday, 9:00 AM – 5:00 PM

Qualifications:

Education: Bachelor’s degree in Dietetics or related field from an accredited program

Licensure & Certification:

Registered Dietitian Nutritionist (RDN) through the Commission on Dietetic Registration – Required

Certified Dietitian Nutritionist (CDN) licensed in New York State – Required

Experience:

Minimum 1 year of experience in clinical nutrition – Required

Experience with frail or elderly populations – Preferred (or must complete training upon hire)

Home care experience – Preferred
