Case Assistant, Weinberg Older Adult Center (OAC)

Job Summary :

The Weinberg Older Adult Center (OAC), Case Assistant plays a vital role in providing support and assistance to older adults in various aspects of their lives. This position involves working closely with our older adults/participants to receive the care, resources, and services they need to maintain their well-being and independence. The primary responsibilities include conducting assessments/intakes, coordinating services, advocating for clients, completion of administrative tasks (including but not limited to, brief reports, trackers, updated client information, etc.), and providing community resources and referrals.

The Weinberg Older Adult (OAC) Case Assistant is instrumental in furthering Bronx House’s Older Adult Center program mission to enhance their quality of life, maintain independence, and provide a better life for everyone.

Staff reports to the Weinberg Older Adult Center Program Director.

Duties and Responsibilities:

  • Conduct comprehensive multidimensional psychosocial assessments within 30 days of enrollment, using the Health Indicators Survey, NSI, and PHQ-9 tools.
  • Develop individualized Service Plans and review them formally every six months.
  • Maintain regular contact (in-person or by phone) to monitor progress, address new needs, and update Service Plans.
  • Coordinate referrals for healthcare, housing, transportation, nutrition, social activities, etc.
  • Advocate for clients’ access to benefits and entitlements (e.g., Medicaid, Medicare, SNAP).
  • Enter detailed case notes in the electronic case management system within four business days of each contact.
  • Submit monthly contact logs, quarterly service summaries, and required DFTA reports to the Program Director.
  • Track all paperwork and applications, ensuring completeness and accuracy.
  • Maintain detailed and accurate logs for all clients; maintain client files.
  • Engage older adults in-center programs, daily lunch services, and off-site activities.
  • Conduct outreach to recruit new participants and follow up on no-show appointments.
  • Respond promptly to crisis situations (medical, housing, mental health), providing short-term intervention and referrals.
  • Alert the Program Director immediately for high-risk or emergency cases.
  • Adhere to agency policies, including COVID-19 safety guidelines, confidentiality requirements, and DFTA Professional Standards of Conduct.
  • Participate in weekly team meetings, case conferences, and ongoing training.
  • Understand and implement services in full accordance with DFTA Standards of Operations and Scope of Services.
  • Perform additional tasks as assigned by the Program Director to meet evolving program needs.

Performance Targets:

  • Deliver at least 750 case assistance service notes per program year (average of four hours of case assistance per program day or 15 case notes per week).
  • Achieve 100% compliance with DFTA recordkeeping and reporting timelines.

Supervisory Responsibilities: None

Qualifications:

  • Bachelor's degree in social work, human services, psychology, gerontology, or related field preferred.
  • Bilingual English/Spanish, Preferred.
  • Previous experience working with older adults, preferably in a case management or social services setting.
  • Knowledge of aging issues, including physical and mental health challenges, caregiving, elder abuse, and end-of-life care.
  • Strong interpersonal skills and the ability to communicate effectively with diverse populations, including older adults, family members, and interdisciplinary team members.
  • Excellent organizational skills and attention to detail, with the ability to manage multiple tasks and prioritize workload effectively.
  • Proficiency in computer applications, including Microsoft Office and case management software.
  • Compassion, empathy, and a genuine commitment to advocating for the well-being and dignity of older adults.
  • Ability to work independently and collaboratively in a fast-paced environment while maintaining professionalism and confidentiality.
  • Flexibility to adapt to changing priorities and client needs.
  • Access to reliable transportation required.
  • Ability to pass verbal/written proficiency tests.
  • Weekend and Holiday work is required as requested per program needs.
